Reversible corticospinal tract hyperintensities in neurologic Lyme disease.

چکیده
A 60-year-old man presented with an ataxic and spastic gait with paraparesis, having reported a tick bite. Brain MRI depicted bilateral corticospinal tract hyperintensities (figure). He had a mild lymphocytic meningitis (12 leukocytes/mm, proteinorachia 1.48 g/L). ELISA was positive for Lyme disease in serum (immunoglobulin G [IgG] 176 UI and immunoglobulin M,4 UI) and CSF (IgG 300 UI) with an intrathecal synthesis (index of CSF/serum-specific antibodies equal to 16 using ELISA [normal value ,1.5]). Western blot was positive for Lyme-specific antibodies in serum (18, 22, 32, 41, 60, and 75 kDa) and CSF (41 and 60 kDa). Syphilis serology was negative. After treatment (IV ceftriaxone 2 g daily for 4 weeks), gait improved and brain MRI was normal. This MRI pattern has been previously reported in amyotrophic lateral sclerosis and in metabolic diseases (Krabbe disease, X-linked adrenoleukodystrophy, cerebrotendinous xanthomatosis) or infectious diseases (human T-cell lymphotropic virus 1).

متن کاملNormal findings on brain fluid-attenuated inversion recovery MR images at 3T.
BACKGROUND AND PURPOSE Fluid attenuated inversion recovery (FLAIR) MR imaging of the brain has become a routine tool for assessing lesions in patients with suspected neurologic disorders. There is growing interest in 3T brain FLAIR MR imaging but little normative data are available. The purpose of this study was to evaluate the frequency and topography of cerebral hyperintensities seen with FLA...

BACKGROUND AND PURPOSE We hypothesized that diffusion tensor fiber tractography would be affected by intracranial arteriovenous malformation (AVM). The purpose of the present study was to evaluate the influence of intracranial AVM on corticospinal tract and optic radiation tractography. MATERIALS AND METHODS The subject group comprised 34 patients with untreated intracranial AVM. Hemorrhage w...
متن کاملThe topography of brain microstructural damage in amyotrophic lateral sclerosis assessed using diffusion tensor MR imaging.
BACKGROUND AND PURPOSE ALS leads to macrostructural (ie, cortical atrophy and hyperintensities along the corticospinal tract) and microstructural (ie, gray matter intrinsic damage) central nervous system abnormalities. We used a multimodal voxelwise imaging approach to assess microstructural changes independent of macrostructural volume loss in patients with ALS compared with HCs. MATERIALS A...

Objective measures of neurologic function were used to assess response to treatment in patients with late Lyme borreliosis. Neurophysiologic evidence of peripheral neuropathy was present in 64 of 137 patients tested. Measures of distal axon function (sensory amplitude and conduction velocity, motor terminal latency) were most affected. Repeat studies following 60 patients receiving antimicrobia...
